Some 2 million Scots are living with at least one long term condition.  That is around 40% of the population.  So, the majority of us will know someone who is living with a long term condition.

However, recent research found that only 45% of the population are aware they know someone living with a long term condition.

Therefore, there is a need to raise awareness and encourage self management so that life can be better for the many people who are living with long term conditions.
